For FY26/27 The Florida Flex team will publish new MBQIP resources on this page every month to support your required reporting and general quality improvement. Each month’s release will include targeted tools, checklists, data tips, and guidance designed to streamline documentation, improve accuracy, and help you meet reporting deadlines.

    How This Resource Can Support Measure Improvement

    Hybrid Hospital-Wide Readmissions | Telligen

    The Hybrid Hospital-Wide Readmission Measure is a hospital-level, all-cause, risk-standardized measure that evaluates unplanned readmissions occurring within 30 days of discharge from acute care hospitalization.

    Featured Tool: PIP Worksheet (p.15)
    Purpose: Establish clear, measurable, and achievable improvement targets that align with organizational priorities and performance goals and enable tracking as part of a Quality Improvement project.

    Example SMART Goal:
    Reduce the MBQIP Hybrid Hospital-Wide Readmission Rate from 12.5% to 10.5% by June 30, 2027, through enhanced discharge planning processes, comprehensive medication reconciliation, patient and caregiver education, and timely post-discharge follow-up.
